What is the Foreigner Physical Examination Form?
The Foreigner Physical Examination Form is the official health form that foreign applicants must complete for study in China. A qualified hospital fills and stamps it. The form is used for your application and later for your visa and arrival checks.

Print both pages clearly on A4 paper. Do not edit or change the form.
Choose a recognised hospital
Go to a government or well-known hospital that can carry out all tests and put an official stamp.
Fill in your personal details
Write your name exactly as on your passport. Attach your recent passport-size photo in the photo space.
Complete all tests
The doctor will fill in the medical history, examination and test sections. This usually includes a general check-up, chest X-ray, ECG and blood and other lab tests listed on the form.
Get signatures and stamps
The doctor must sign, and the hospital must stamp the form, including the seal across the photo where required.
Check every page
Make sure no field is empty and nothing is overwritten. Ask the hospital to correct any mistake and stamp it.
Scan and keep the original
Scan all pages in colour as one clear PDF. Keep the original forms and lab reports for verification and arrival.
The physical examination result is usually valid for a limited period, often about six months. Take the test close to your application, not too early.
Missing tests, missing stamps or an unsigned page can make the form unacceptable. Check the list of items on the form before leaving the hospital.
